    In my short experience it is much better than conventional balloon angioplasty specially in the infra popliteal arterial tree.

    Hi Shimam,
    to be hones we use this method quite frequently in patients in whom we do not manage to perform intraluminal recanalization or to get back to the true lumen from above. You could also puncture the popliteal artery but this involves turning the patient and is more cumbersome. When I first performed this type of and approach (after having learned this from Andrej Schmidt, Leipzig), I was astonished how fast this goes.
  • Which vessels are most likely to bleed?

    Supposing that the CT was not helpful in locating the bleeding artery, are there certain pelvic branch vessels that are more likely to bleed than others after trauma?  In other words, are there certain pelvic branch vessels we should look at particularly carefully when doing the angiogram?

    For instance, with an upper GI bleed, we are already suspecting the GDA to be the culprit and will take a good hard look at it during the angiogram.

    We liked this case because it presents a clear-cut example of the value interventional radiology can have in trauma situations. We have been working on streamlining our trauma protocols to ensure that these patients are quickly brought to IR for angiogram and possible intervention. How have other instituitions ensured that the team is ready and available for trauma during daytime and evening hours so these critical patients dont wait?

    Interesting approach. I am getting superb result of dealing with intrastent neo-intimal hyperplasia with recoil of stenosis after angioplasty by deploying another same size stent inside. Even bare stents inside bare stents are giving a very good long term results. Covered stent deployment (Viabhan and Fluency) are giving simply superb results. If it works in dialysis access, it should also work in the SFA, I guess, especially Viabhan.

    Noli nocere!  Removal of the catheter was unlikely to solve patient's problem with right arm swelling and recanalizing the
    outflow from the right arm AV fistula.  Of note, I do not see the stent in the SVC, it ends at the distal end of the right brachiocephalic vein. I would try to open the outflow into the SVC first, if impossible, I would not remove the perm catheter fragment.
